BackgroundCumulativebloodpressure(BP)exposuremayadverselyinfluencemyocardialfunction,predisposingindividualstoheartfailurelaterinlife.
ObjectivesThisstudysoughttoinvestigatehowcumulativeexposuretohigherBPinfluencesleftventricular(LV)functionduringyoungtomiddleadulthood.
MethodsTheCARDIA(CoronaryArteryRiskDevelopmentinYoungAdults)studyprospectivelyenrolled5,healthyAfricanAmericansandwhitesinand(baseline).Attheyear25examination,LVfunctionwasmeasuredby2-dimensionalechocardiography;cardiacdeformationwasassessedindetailbyspeckle-trackingechocardiography.WeusedcumulativeexposureofBPthroughbaselineanduptotheyear25examination(millimetersofmercury×year)torepresentlong-termexposuretoBPlevels.LinearregressionandlogisticregressionwereusedtoquantifytheassociationofBPmeasuredrepeatedlythroughearlyadulthood(18to30yearsofage)uptomiddleage(43to55years).
ResultsAmong2,participants,cumulativeBPmeasureswerenotrelatedtoLVejectionfraction;however,highcumulativeexposuretosystolicbloodpressure(SBP)anddiastolicbloodpressure(DBP)wereassociatedwithlowerlongitudinalstrainrate(bothp0.).Fordiastolicfunction,highercumulativeexposurestoSBPandDBPwereassociatedwithlowearlydiastoliclongitudinalpeakstrainrate.Ofnote,higherDBP(perSDincrement)hadastrongerassociationwithdiastolicdysfunction
